Established in 1979, Community Hospice of Northeast Florida is the only locally based, nonprofit hospice provider serving residents of Baker, Clay, Duval, Nassau and St. Johns counties. Each day, 940 employees and 930 volunteers help approximately 1,300 patients and their families live better with advanced illness.
