Empowering Jacksonville's Elderly Population Through Comprehensive Care and Program Growth

August 24, 2023 (Jacksonville, Fla.) – PACE Place, the Program of All-Inclusive Care for the Elderly, is proud to announce its remarkable growth in Jacksonville, Fla. Committed to enhancing the lives of the elderly community who are 55 and older, PACE Place has become a leading force in providing all-encompassing and empathetic support for seniors, and with that comes the growth of this program to include 100 more participant open spots to fill.

 

PACE Place, the only PACE program in Northeast Florida, is nationally recognized and offers integrated healthcare, social support, and wellness services to eligible seniors. By enabling elderly individuals to live independently in their homes and communities, PACE Place stands as a beacon of support, fostering a sense of dignity and well-being among its participants.  Along with the medical advantages, a large range of recreational activities are offered including movie nights, gaming nights, beauty school days, gardening groups, storytelling, off-campus day trips, karaoke nights, and many more.

 

The heart of PACE Place's mission is to empower Jacksonville's elderly population to age gracefully and comfortably while maintaining their independence. The program focuses on a person-centered approach that tailors care plans to meet the unique needs and preferences of each participant, encompassing medical, social, emotional, and functional aspects of their well-being.

 

As PACE Place builds its program, it remains dedicated to delivering top-notch, holistic care for the elderly. The program's interdisciplinary team of healthcare professionals collaborate to provide comprehensive care that addresses every aspect of the seniors' health and happiness. Additionally, transportation is provided to and from The PACE Place and to approved medically necessary appointments. The growth of PACE Place in Jacksonville is a testament to its positive impact on the lives of the elderly. PACE Place can now serve an even larger number of seniors in the Northeast Florida region, ensuring more elderly individuals have access to the high-quality care and support they deserve.

About The PACE Place

PACE Place is a part of the national network of programs called Program of All-Inclusive Care for the Elderly (PACE), providing seniors and their families with comprehensive care, nutrition, rehabilitation, transportation, and supportive services. Approved by the Centers for Medicare and Medicaid and the State of Florida, PACE Place coordinates all care and services, including transportation, to support caregivers and their loved ones. Care coordination for the PACE’s Partners of Northeast Florida’s program, the first of its kind in North Florida, is delivered at The PACE Place, located at 5450 Ramona Boulevard, Jacksonville, FL 32205, and at the participant’s home. At PACE Place, we are honored to serve as care providers, helping our participants maintain their independence and receive the support they need to remain at home. Our joint venture, PACE Partners of Northeast Florida, a collaboration between Community Hospice & Palliative Care and Aging True, ensures comprehensive care coordination for the PACE program at both The PACE Place and participants' homes. As a result, our participants lead full and meaningful lives.
