September is National PACE Month

Open House Invitation

 

September 6, 2023 (Jacksonville, Fla.) – To kick off National PACE Month, PACE Place invites interested families to visit and see firsthand why Programs of All-Inclusive Care for the Elderly (PACE) are the ideal option for keeping seniors independent in their homes and cared for. The theme for the month is “PACE: Where You Belong.”

 

On Friday, September 15, 2023, from 9:30 a.m. to 11:30 a.m. PACE Place, located at 5450 Ramona Blvd., Jacksonville, FL 32205, is hosting an open house and invites the public to come out and see the facility, meet the staff, and learn all about the PACE program. Guests will enjoy music, a coffee bar, snacks, and tours of the facility will be available.

 

PACE Place, the only PACE program in Northeast Florida, is nationally recognized and offers eligible seniors integrated healthcare, social support, and wellness services. By enabling elderly individuals to live independently in their homes and communities, PACE Place stands as a beacon of support, fostering a sense of dignity and well-being among its participants. Along with the medical advantages, an extensive range of recreational activities are offered, including movie nights, gaming nights, beauty school days, gardening groups, storytelling, off-campus day trips, karaoke nights, and many more.

PACE Place is a part of the national network of programs called Program of All-Inclusive Care for the Elderly (PACE), providing seniors and their families with comprehensive care, nutrition, rehabilitation, transportation, and supportive services. Approved by the Centers for Medicare and Medicaid and the State of Florida, PACE Place coordinates all care and services, including transportation, to support caregivers and their loved ones. Care coordination for the PACE’s Partners of Northeast Florida’s program, the first of its kind in North Florida, is delivered at PACE Place, located at 5450 Ramona Boulevard, Jacksonville, FL 32205, and at the participant’s home.
