This award recognizes hospices that consistently provide outstanding care and support to patients and their families.
The Hospice CAHPS Honors Award highlights the commitment to patient satisfaction and quality care. To earn this accolade, Community Hospice & Palliative Care surpassed the national average score in the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey, particularly in the 'Willingness to Recommend' category. This rigorous evaluation process, which included 23 other quality measures compared to national benchmarks, underscores our dedication to excellence.
Community Hospice & Palliative Care is the only hospice provider in Northeast and North Central Florida to receive this distinction. Phillip Ward, President, and CEO, credits the entire team's dedication and commitment to extraordinary care for achieving this honor.
